Make it easy for visitors to become business leads by adding service inquiry fields to your form. They can choose the service they’re interested in, share their details, and even book a free consultation with an appointment field if you’d like.

Service inquiry forms help you collect new leads, offer intro meetings, and keep everything organized. Inquiries go straight to your Wix Inbox, while booked appointments show up on your calendar, so you’re always ready to connect and grow your business.

When you add a service inquiry field to your form, you are prompted to install Wix Services if it is not already installed.

Step 1 | Add a form to your site

Add a form to your site to gather valuable insights while allowing your visitors to submit inquiries, subscribe to newsletters, make purchases and more.

Go to your editor.
Click Add Elements on the left side of the editor.
Click Contact & Forms.
Choose from the following options: Create New Form, Add Existing Form, or Premade Forms.
Click the desired form to add it to your page.

Step 2 | Add service inquiry fields to your form

Add service inquiry fields to your form so visitors can pick the services they want. You can also let them book an intro meeting with an appointment field, making it easy to collect leads and stay organized.

To add service inquiry fields to your form:

(If you selected Create New Form) Skip to step 3 below.
Click the form you added and then click Edit Form.
Click Service inquiry and choose what you want to add:

Appointment

Click Appointment.
- Click the relevant tab to edit the General or Advanced appointment field settings:
- Customize the following options under General:
  - Field title: Give a short title that tells people what you are asking for in the field.
    Tip: Click the More Actions icon to hide the field title.
  - Appointment name: The name that appears on the booking calendar and for client communication.
- Duration: Select the appointment duration from the drop-down menu.
- Meeting format: Choose where the session takes place:
  - Phone call: Meet over the phone and add a description for clients to know how the session will take place. Note: Add a Phone number field to the form for phone call meetings.
    - Online: Connect a video conferencing account and meet online. Note: The meeting link is sent via email so make sure to add an Email field too.
    - In person: Describe the location in the field provided or choose the location from the drop-down.
  - Staff: Select the staff member hosting the appointment. You can click Manage availability to edit their hours.
    Note: If you assign a staff member with booking permissions, they should sync their calendar. The calendar reflects the assigned staff member's availability.
  - Required field: Enable the toggle to make this appointment field mandatory. If enabled, make sure there is availability, or clients won't be able to submit bookings due to unavailable slots.
  - Field description: Add explanation if needed.
  - Personal information (PII): Protect personally identifiable information (PII) such as addresses and passport details, with an additional layer of protection.
- Advanced: Choose from the following options under Advanced:
  - Booking preferences: Choose if appointments are automatically or manually confirmed.
  - Calendar sync: Make sure the personal calendar is synced to block off unavailable times and avoid double bookings.
  - Field key: If needed, edit the field key used to reference the form field in code. You cannot edit this field key after you save this form field.
Click Save.
