Currently, you cannot add the 123 Form Builder app in ADI. If you want to add it, you should head to the Editor and follow the instructions here.
If you have a 123 Form Builder app coupon, you can use it from the Editor.
Warning:
If you decide you want to go back to ADI after moving to the Editor, follow the instructions here. If you choose to return to ADI, you will not see any of the changes made in the Wix Editor.
